Medical Massage may be covered by your insurance company, however, we are only contracted with the following insurance companies:

  • Auto PIP Claims

  • Labor & Industries Claims

  • Premera/Blue Cross

  • Kaiser (Access/PPO, Option/PPO, and HMO/CORE/PPO)

  • Lifewise

  • First Choice

  • Coordinated Care


If your insurance company is not listed, no problem, we will bill them for you! Please just understand there is a process to this and we still need a referral from your provider before or at your first appointment. we are happy to assist just give us a call or send us an E-mail.

Important Information Regarding Insurance Claims:

  • Please check with your insurance company regarding your benefits prior to your visit. If we bill your insurance company and they deny your claim, then you are required to pay for services out of pocket.

  • In order to bill your insurance company for medical massage, you must have a prescription from your doctor or other medical professional that includes: date issued, diagnosis and ICD-10 code, plus the recommended frequency and duration of treatment.

  • If you do not have a prescription, you will be required to pay out of pocket for the visit. If you obtain a prescription after the service has been performed and the prescription is back-dated, we can give you a refund after we receive payment from your insurance company.

  • Any balances not covered by insurance are due within 60 days of the initial billing unless other arrangements have been made with our billing department.

  • Please arrive around 15 minutes early for your visit to complete the necessary paperwork, including the 'Client Intake" form below. Please print and fill out prior to your appointment.
